FIRST VISIT

During your first visit, we will take some time to get to know you and to introduce you to our office. We will discuss in detail your dental needs and desires so that we may provide you with the highest possible level of care. You will spend one on one time with Dr. Erickson and our staff for a comprehensive oral exam in which we will evaluate the health of your teeth and gums. We will take the necessary x-rays to help make an assessment of your oral condition in order to design a customized dental health plan tailored to your needs. We will do this with your input and include you in the diagnostic loop. While you are here for your initial visit, Dr. Erickson’s time is devoted to you; he will be spending the time to get to know you and your specific concerns. If it is discovered that you need any treatment, a treatment plan and estimate will be prepared and presented to you. You will have the chance to review the recommended treatment, and address any questions or concerns prior to scheduling any procedure.

Please assist us by providing the following information at the time of your consultation:

• A list of your current medications including all natural supplements.
• If you have dental insurance, bring the card or member information.
  This will save time and allow us to help you process any claims.
• IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the first visit.
• Please alert the office if you have a medical condition or if you are on any medication. (i.e., diabetes, high blood pressure, artificial heart valves, artificial joints, rheumatic fever, heart conditions, etc.)

X-rays

If you have x-rays from a previous dentist or physician, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
